Terms & Conditions

I hereby state that I am the owner/person-in-charge of the patient.  I consent to the patient being assessed by our qualified staff to determine if Hydrotherapy or other treatment(s) is/are suitable for the patient.  Once suitability has been confirmed, I consent to the patient having on-going Hydrotherapy and/or other treatment(s) with a qualified staff.  This may be assisted or unassisted.

I understand that:

• Assessment involves the identification of any reasons not to undertake Hydrotherapy and/or other treatment(s);
• Our staff may request another assessment part-way through a treatment program, if there is any concern for the welfare of the patient;
• All animal handling carries inherent risks to the animal, handler, and our staff;
• Geelong Canine Fitness may refuse to handle the patient, if the situation is deemed unsafe for any person, animal and/or third-party, and/or when important information, such as x-rays and veterinary reports, are not made available when requested;
• Referral to my usual veterinarian or another practitioner maybe advised;
• Late cancellation fee may apply, if appointment is cancelled within 24 hours of the  appointment time;
• Other rehabilitation strategies or changes in my patient’s lifestyle maybe required for successful outcomes; and
• While reasonable care is taken, any form of rehabilitation carries inherent risk of adverse effect.

I agree:

• To follow the staff instruction regarding safe handling and practices;
• To pay in full for all assessment and/or treatment(s) provided on the day of service;
• That Geelong Canine Fitness can share and request patient information including history, radiographs, medications, and treatment with other practitioners where I have given written consent; and
• That I have read, understand the Terms outlined above.

I give consent to pictures and/or videoes of my patient being posted on the Geelong Canine Fitness social media.
